From the document: "This Instruction implements Air Force Policy Directive (AFPD) 41-1, Health Care Programs and Resources, and AFPD 41-2, Medical Support. This instruction applies to the United States Space Force (USSF), Regular Air Force (Reg AF), AF Reserve (AFR) and Air National Guard (ANG) and Department of the Air Force (DAF) civilian employees. This Instruction requires the collection and or maintenance of information protected by the Privacy Act of 1974 authorized by Title 10 United States Code, Section 9013, Secretary of the Air Force. The applicable SORN F036 AF PC C, Military Personnel Records System, is available at: http://dpclo.defense.gov/Privacy/SORNs.aspx. Ensure all records created as a result of processes prescribed in this publication are maintained in accordance with DAFMAN 90-161, Publishing Processes and Procedures, and disposed of in accordance with the air force records disposition schedule located in the Air Force Records Information Management System. System of Records notice F036 AF PC C, Military Personnel Records System, applies. Refer recommended changes and questions about this publication to the OPR using the DAF Form 847, Recommendation for Change of Publication; route through the appropriate functional chain of command. This publication may be supplemented at any level, but all supplements must be routed through the Office of Primary Responsibility (OPR) of this publication for coordination prior to certification and approval. The authorities to waive requirements in this publication are identified with a Tier (“T-0, T-1, T-2, T-3”) number following the compliance statement. See DAFMAN 90- 161, Publishing Processes and Procedures, for a description of the authorities associated with the Tier numbers. Submit requests for waivers through the chain of command to the appropriate Tier waiver approval authority, or alternately, to the requestor’s commander for non-tiered compliance items. In addition, copies of all waiver requests submitted for requirements in this instruction will be provided to the parent Major Command Medical Readiness Office (MAJCOM/SGX), regardless of Tier. The use of the name or mark of any specific manufacturer, commercial product, commodity, or service in this publication does not imply endorsement by the Air Force. Note: The term MAJCOM, when used in this publication, refers to all MAJCOM, Field Operating Agencies (FOA), Direct Reporting Units (DRU), Field Comm (FLDCOM), Air Reserve Component and ANG organizations, unless otherwise indicated."
